You may print the form out and send it along with the membership amount, or complete the form on-line, forwarding the membership fee to KQHRA, P O Box 228, Eureka, KS 67045.  Individual membership is $40.  Husband and wife can each become voting members for $60, or you can become a lifetime member for only $250 each.   Youth membership (18 & under, no vote) is $5.
